3/11/2023 EnchantaVaughn Approved 1.Behavior: arrival to the home, Jahsim was confronted and sent to have one on one with his QMHP-C. Jahsim stated he didn't get much sleep the night before and was not in a good mood. Jahsim was encouraged to lose the attitude and get his work done before he's penalized for his actions. Jahsim have struggled to ignore distractions and finds himself off task and interrupting others who are trying to learn. 2.Behavior: Jahsim participated in the individual session and presented withdrawn behavoirs as he was paying attention to his brother complete his homework assignments with his mother and continuing to focus on his negative behaviors such as following directives within the home setting. 1.Intervention: QMHP-C trained Jahsim in taking steps to prevent problems including displaying positive home behaviors. QMHP-C refocused Jahsim as he was observed becoming fidgety and restless. QMHP-C will review and give feedback regarding ADHD and its causes and affect toward Jahsim well-being and academic functioning. QMHP-C will provide Jahsim with support and encouragement. QMHP-C will monitor, observe, and redirect Jahsim behaviors as needed. QMHP-C will build rapport. QMHP-C will explore positive self-talk, use instruction modeling, and increase social communication skills with positive reinforcement role play activity. 2. Intervention: QMHP-C used positive regard as he told Jahsim that he appeared to have positive home management in class earlier in the day. QMHP-C informed Jahsim of more acceptable and expected responses to situations that would normally lead Jahsim to be triggered and display inappropriate, negative behaviors while in the home. QMHP-C assisted Jahsim to behave in more positive ways by addressing the conflict he was facing with his mom. QMHP-C informed Jahsim of replacement behaviors in efforts to decrease attention seeking behaviors Jahsim often is involved in. QMHP-C modeled appropriate ways for Jahsim to get positive attention from his family. QMHP-C inquired of Jahsimsâ current mood. QMHP-C utilized active listening skills as Jahsim spoke of his current mood. QMHP-C will educate Jahsim on behavior modification to help Jahsim identify what behavior he needs to decrease and avoid daily. QMHP-C will utilize behavior modification system to aid Jahsim with following directions given by authority figures. QMHP-C will meet with Jahsim daily to discuss progress and expectations that are working and not working in the home weekly. 1.Response: Jahsim communicated thoughts and concerns about thinking positively at all times. Jahsim expressed that he tries to think highly of himself. Jahsim communicated that he feels that he will be better behaved, due to consistent support in the home with mom and seeing QMHP-C 4x a week. Jahsim showed interest in building interpersonal relationships with peers and authority to increase support system. Jahsim expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity. Jahsim learned to avoid negative and defiant behavior and comply to rules. Jahsim was seen following directions. Jahsim joined session to utilize and learn new interventions and strategies. 2.Response: Jahsim expressed appreciation to QMHP-C by smiling and giving good eye contact. Jahsim expressed that he should receive an incentive for his display of desirable behaviors throughout the day. Jahsim did not engage in any disruptive activities or behaviors. Jahsim was able to maintain his composure when he was confronted with the potential for conflict. Jahsim was observed engaging in several conflict resolution skills by walking away and telling an adult first. Jahsim verbalized that heâs able to focus better when it is quieter and more peaceful. Jahsim took his mind from thinking and doing to a place of concentration, which allowed him to focus on one thing at a time instead of being all over the place and getting easily distracted. Jahsim demonstrated more acceptable responses, avoiding anger provoking situations in the home and community.
